Referring a Patient for Admission

Patients are typically referred to RUSH Specialty Hospital by their physicians or discharge planners (social workers, case managers) at the acute care hospital or other health care facility where they are being treated. In some cases, patients may come to us directly from home.

To facilitate admission to our hospital, please contact our admission office at (872) 298-9260 or have the discharge planner fax the following documentation to (312) 264-6161:

  • History and physical
  • Physician and nursing notes
  • Physical, occupational and/or speech therapy notes
  • Medications
  • Lab reports
  • Face sheet

A physician at RUSH Specialty Hospital will carefully review your medical records and determine whether admission is appropriate based on your medical history and rehabilitation needs and the services you require. Your ability to participate in and benefit from three hours of therapy a day, five days per week, (or 15 hours a week, as tolerated) and achieve individual rehabilitation goals will also be reviewed.

Patients admitted to RUSH Specialty Hospital – Inpatient Rehabilitation must also require at least two of the following treatment disciplines: physical, occupational and/or speech therapies.

Our clinical liaisons will help coordinate your transfer to our hospital. If possible, they will meet with you and your family to discuss admission, answer questions and help expedite the review process.

Due to the significant risk posed by medical identity theft, we take precautions to safeguard personally identifying information. This includes requiring admissions staff to request photo identification upon arrival.

As a reminder, all patients should take steps to protect their insurance cards, Medicare and Social Security numbers during their stay.
